Contractors, Suppliers, and Vendors are requested to complete and return the forms listed below and attach the requested documentation before your business will be considered as an approved vendor or eligible for payment for goods and/or services performed on behalf of any community association or in relation to management services requested on their behalf by Innovative Property Management, LLC.

- Request for Taxpayer Identification Number
- Liability Insurance Certificate from your Insurance Agent with a minimum of $1,000,000.00 in Liability Coverage
